1. General Participation Waiver
I, the undersigned, hereby give permission for myself/my child (the “Participant”) to participate in the robotics class. I understand and acknowledge that participation in this program may involve physical activity and other activities that carry a risk of injury. I assume all risks associated with participation in the program and release Burning Gear Robotics, its staff, volunteers, and agents from any and all liability for injury, loss, or damage to person or property that may occur during the program.
2. Emergency Medical Consent
In the event of a medical emergency, I authorize the program staff to seek appropriate medical care for the Participant. I understand that I am responsible for all costs associated with such medical care.
3. Photo and Video Release
I understand that Burning Gear Robotics may take photographs and/or videos during the program for promotional, educational, and informational purposes. I grant Burning Gear Robotics permission to use the Participant’s image, likeness, or voice in any medium without compensation. If I do not wish for images or videos of the Participant to be used, I will notify the program in writing prior to the start of the program.
4. Indemnification Clause
I agree to indemnify and hold harmless Burning Gear Robotics, its staff, volunteers, and agents from any claims, actions, or suits arising from or related to the Participant’s involvement in the program.
5. Acknowledgment of Policies
I acknowledge that I have received, read, and understood the program’s policies and agree to adhere to them. I understand that failure to follow these policies may result in removal from the program without refund.
6. Assumption of Risk
By checking this and enrolling, I confirm that I have disclosed any medical, physical, or emotional conditions that could affect the Participant’s ability to safely participate in the program. I assume all risks associated with any disclosed or undisclosed conditions.
7. Missed Classes
If a class is missed due to personal reasons, no make-up class will be offered.
8. Refund Policy
No refund will be offered after the first class has finished.
